Scientists have used artificial intelligence together with MRI brain scans to measure, for the first time, how quickly fluid moves through the brain's cleaning system, known as the glymphatic system.
This system removes waste products from the brain, including amyloid beta, a protein connected to Alzheimer's disease. Until now, it was very hard to measure how fast this cleaning process happens inside a living brain.
The researchers used physics informed neural networks, a type of AI, to study MRI data and estimate fluid speed. They discovered two very different routes: fluid moves quickly through spaces near the brain's outer surface, but about 50 times more slowly through the deep tissue inside the brain.
Scientists say this discovery could eventually help doctors detect circulation problems linked to Alzheimer's, aging, and concussions, possibly allowing earlier screening before serious symptoms appear.

To solve this problem, the research team applied physics informed neural networks, a class of AI that incorporates known physical laws into its predictions, to analyze MRI data and calculate fluid flow velocities. Their results, published in Science Advances, revealed two distinct clearance routes with dramatically different speeds: fluid travels rapidly through the brain's outer perivascular spaces but roughly 50 times more slowly through deep brain tissue.
The researchers say this sharp contrast could eventually allow clinicians to identify circulation abnormalities associated with aging, Alzheimer's disease, and concussions well before patients develop noticeable symptoms, potentially opening a path toward earlier intervention.

To close that gap, the research team deployed physics informed neural networks, a class of machine learning models that embeds governing physical equations directly into the training process, to interpret MRI data and derive fluid velocities across brain regions. Published in Science Advances, the findings revealed a striking bimodal pattern: fluid traverses the brain's outer perivascular spaces rapidly, while flow through deep parenchymal tissue proceeds roughly fiftyfold more slowly, a disparity the authors argue reflects fundamentally distinct clearance mechanisms operating at different depths.
The authors suggest this newly quantified contrast could eventually serve as a noninvasive biomarker, enabling clinicians to detect glymphatic dysfunction associated with aging, neurodegeneration, and traumatic brain injury before overt cognitive symptoms emerge, potentially shifting the diagnostic window for Alzheimer's disease and related conditions well earlier in their progression.
