Sleep apnea is a health problem. When people sleep, their breathing stops for short moments, again and again. They feel very tired in the day.
Many people use a machine called CPAP. It pushes air into the nose with a mask. Some people do not like the mask and stop using it.
Now scientists have tested a pill. People take it one time each night. In the test, the breathing stops happened about 44 percent less often.
The test had 646 adults. It lasted six months. The company wants to ask the government for permission to sell the pill.

Level 2 — Elementary
Obstructive sleep apnea happens when the muscles around the airway relax during sleep and the airway partly closes. Breathing stops many times each hour, oxygen levels fall, and people wake up tired. The condition is linked to high blood pressure and heart problems.
The standard treatment is a CPAP machine, which blows air through a mask to keep the airway open. It works well, but many patients find the mask uncomfortable and give up using it.
A new drug called AD109 offers another option. It is a single nightly tablet that combines two medicines, aroxybutynin and atomoxetine, and it works on the nerves and muscles that keep the airway open instead of using air pressure.
In a phase 3 trial at 69 sites in the United States and Canada, 646 adults took the drug for six months. Breathing interruptions fell by about 44 percent. More than 40 percent of patients moved into a less severe category, and 18 percent reached complete disease control. The company Apnimed plans to apply to the US Food and Drug Administration.

Level 3 — Intermediate
Obstructive sleep apnea affects a very large share of adults worldwide, yet its treatment has barely changed in a generation. Continuous positive airway pressure, universally abbreviated to CPAP, is highly effective when used, but adherence is the persistent weakness: a substantial proportion of patients abandon the mask within the first year, and those patients have had no pharmacological alternative at all.
AD109 is designed to address that gap. Rather than splinting the airway open with pressurised air, the drug targets the neuromuscular control of the upper airway, combining aroxybutynin with atomoxetine to increase the activity of the muscles that prevent collapse during sleep. It is taken once at night.
The phase 3 programme enrolled 646 adults with mild to severe apnea across 69 sites in the United States and Canada, all of whom either could not tolerate CPAP or had declined it. Over six months, the apnea-hypopnea index, which counts breathing interruptions per hour, fell by roughly 44 percent. Oxygen levels improved, more than 40 percent of participants shifted into a less severe disease category, and 18 percent achieved complete control.
Apnimed intends to submit a new drug application to the Food and Drug Administration. If approved, AD109 would become the first oral treatment for the condition, though clinicians caution that a 44 percent reduction is meaningfully less than what a well-used CPAP machine delivers, and the drug is likely to be positioned for patients who cannot use one.

Level 4 — Advanced
Obstructive sleep apnea has long occupied an awkward position in medicine: a disorder of unmistakable prevalence and well-documented cardiovascular consequences, treated by a device that works superbly in the laboratory and imperfectly in the bedroom. The efficacy of continuous positive airway pressure has never been the difficulty. Adherence has. Trials of CPAP are, in effect, trials of tolerance, and the population that discontinues therapy has until now had nothing else to be offered.
AD109 attacks the problem from a different direction. Upper airway collapse during sleep is not purely anatomical; it reflects a loss of tone in the dilator muscles, particularly the genioglossus, as neural drive falls during non-REM sleep. By combining an antimuscarinic, aroxybutynin, with the noradrenaline reuptake inhibitor atomoxetine, the compound aims to restore that drive pharmacologically rather than to overcome collapse mechanically.
The phase 3 data support the mechanism without overselling it. Across 646 participants at 69 North American sites, six months of treatment produced roughly a 44 percent reduction in the apnea-hypopnea index, accompanied by improved oxygenation, migration of more than 40 percent of patients into a milder severity band, and complete disease control in 18 percent. These are respectable numbers for an oral agent and modest ones next to optimally used CPAP, which can reduce events by considerably more.
That contrast defines the likely clinical niche. A drug that delivers a partial response to a patient who uses it every night may outperform a device that delivers a near-complete response to a patient who abandons it in March. The regulatory question before the Food and Drug Administration, once Apnimed files, is therefore less about whether AD109 beats CPAP than about whether real-world effectiveness, rather than idealised efficacy, is the standard against which a genuinely new therapeutic class should be measured.
